Impaction
The tooth is trapped under gum or bone, or angled into the molar in front of it. It can’t erupt properly and often damages its neighbor.
Pain, swelling or infection
A partially erupted wisdom tooth traps food and bacteria under a flap of gum—leading to repeated infections (pericoronitis).
Crowding
Pressure from erupting wisdom teeth can shift front teeth, undoing years of orthodontics.
Decay you can’t reach
Wisdom teeth sit so far back that brushing and flossing them well is nearly impossible, so cavities—in them and the tooth in front—are common.
Cysts or bone damage

What to expect
Wisdom tooth removal at Spring Valley Dental Care
- Consultation and 3D scan. A CBCT scan shows the exact position of each tooth, its roots and the nearby nerve. Dr. Mady walks you through it and explains the plan, sedation options and cost.
- The day of surgery. You arrive with a driver. An IV is placed (if you chose IV sedation) and the area is numbed thoroughly.
- Removal. The teeth are removed gently; impacted teeth may be sectioned to come out in small pieces through a smaller opening. Typically 30–60 minutes for all four.
- Recovery instructions. Gauze, ice, medication and a soft-food plan. We call you the next day to check in.
- Follow-up. A quick visit about a week later to confirm you’re healing well.

What the first week looks like
- Day 1: Rest. Bite on gauze to control bleeding, ice 20 minutes on / 20 off, take medication as directed. Cool, soft foods only. No straws, smoking or spitting.
- Days 2–3: Swelling peaks, then starts to fade. Continue soft foods; begin gentle salt-water rinses after meals.
- Days 4–7: Most patients return to school or work. Gradually add soft solid foods. Keep the sites clean.
- Week 2 and beyond: Sockets close over the next few weeks. Call us if pain increases after day 3, or if you notice a bad taste or fever.

Does wisdom tooth removal hurt?
The procedure itself is done under local anesthetic and, for most patients, IV sedation administered by Dr. Mady—so you rest through it and remember little. Afterward, expect swelling and soreness for a few days, managed with medication, ice and soft foods. Most patients say the recovery was easier than they feared.
Do I need a referral to an oral surgeon?
Usually not. Dr. Mady is an Associate Fellow of the American Academy of Implant Dentistry with extensive surgical training and a New Jersey IV sedation permit, and he removes wisdom teeth—including impacted ones—in our Maywood office. In rare cases involving unusual anatomy or medical conditions, he will tell you honestly if a hospital setting is safer.
What is the best age to remove wisdom teeth?
The late teens through mid-twenties. Roots are not fully formed, bone is softer, and healing is faster. That said, adults of any age can have wisdom teeth removed safely; recovery may take a little longer.
Can all four be removed at once?
Yes, and most patients prefer it—one sedation visit, one recovery. Dr. Mady will recommend the approach that fits your case and your schedule.
How should I prepare?
If you are having sedation, don’t eat or drink for the number of hours we specify, wear comfortable clothing, and arrange for an adult to drive you home. Stock up on soft foods—yogurt, smoothies, mashed potatoes, soup—and pick up any prescriptions ahead of time. We go over every detail at your consultation.
What is dry socket and how do I avoid it?
Dry socket happens when the blood clot protecting the extraction site is dislodged, exposing bone and causing pain a few days after surgery. Avoid straws, smoking, vigorous rinsing and spitting for the first several days, and follow the aftercare instructions we give you. If pain gets worse instead of better, call us—it’s easily treated.
