Dr. John Mady, DDS, FAGD, AFAAID, leads the practice his father founded in Maywood in 1977. He earned his DDS at New York University, holds a Fellowship in the Academy of General Dentistry (a distinction fewer than 6% of dentists reach), is an Associate Fellow of the American Academy of Implant Dentistry, and is licensed in IV sedation in New Jersey.

Do you take my insurance?

What does “out of network” mean for me?

What if I don’t have dental insurance?
Join our in-office membership plan: $287 per year covers two cleanings, exams and X-rays (a $390 value) with discounts on additional treatment—no deductibles, no waiting periods, no claim forms. New patients can also start with the free limited exam or the $97 exam, X-rays and cleaning.
Will you tell me what my plan pays before treatment?
Yes. Before your visit we verify your coverage, and before any treatment beyond a checkup you receive a written estimate showing what your plan is expected to pay and what you owe. No surprises at checkout.
Does insurance cover implants, cosmetic work or sedation?
It varies. Many plans now cover part of implant treatment; some medical plans cover extractions and sedation for surgery; cosmetic treatment such as whitening and veneers is usually not covered. We check both dental and medical benefits and quote everything in writing.
Can I use my benefits before they expire?
Most dental plans reset on January 1 and unused benefits are lost. If you have remaining coverage or a flexible spending account, fall is the time to schedule—call and we’ll help you plan treatment around your benefits.
