We are in network with many dental plans and accept out-of-network benefits; our team verifies your coverage before you arrive so there are no surprises at checkout.
No insurance? The $287 membership plan covers a full year of cleanings, exams and X-rays. CareCredit, Alphaeon and Cherry financing are available for treatment.

Meet Dr. John Mady
Dr. John Mady, DDS, FAGD, AFAAID, leads the practice his father founded in Maywood in 1977. He earned his DDS at New York University, holds a Fellowship in the Academy of General Dentistry (a distinction fewer than 6% of dentists reach), is an Associate Fellow of the American Academy of Implant Dentistry, and is licensed in IV sedation in New Jersey.

Where do I fill out new patient forms?
After you book—online or by phone—we send you a secure link to complete registration, medical and dental history, and consent forms electronically. It takes about ten minutes. If you prefer paper, arrive fifteen minutes early and we’ll have them ready.
What should I bring to my first appointment?
Your dental insurance card (or a photo of it), a photo ID, a list of medications you take, and any recent X-rays if you have them from a previous dentist. If you are seeing us for a specific problem, jot down when it started and what makes it better or worse.
What happens at the first visit?
A hygienist takes digital X-rays and cleans your teeth; Dr. Mady examines every tooth, your gums and bite, screens for oral cancer, and reviews your X-rays with you on screen. If anything needs attention, you leave with a written plan and cost. Plan on 60 to 75 minutes.
How much does a first visit cost?
New patients pay $97 for a comprehensive exam, full X-rays and cleaning—a $390 value—or nothing for a free limited exam and X-ray focused on a specific concern. Most dental insurance covers new patient exams and cleanings; we verify your benefits before you arrive.
Can you transfer my records from my old dentist?
Yes. Give us the name of your previous office and we’ll request your X-rays and records, which can save you time and radiation.
I’m nervous. Should I say something?
Please do—on the form or when you book. Laughing gas, oral sedation and in-house IV sedation are available, and knowing in advance lets us plan a calmer visit.
Do you see new patients from outside Maywood?
