Pain that lingers
Sensitivity to hot or cold that keeps aching for 30 seconds or more after the trigger is gone—a sign the nerve is inflamed rather than just sensitive.
Throbbing or spontaneous pain
A toothache that wakes you up, comes on without a trigger, or throbs with your heartbeat usually means infection inside the tooth.
Swelling or a gum pimple
Swelling of the gum, face or jaw, or a small bump on the gum that drains, means an abscess—call the same day.

The procedure
What actually happens during a root canal
- Diagnose. Exam, digital X-rays and, when helpful, a 3D CBCT scan to see every canal and the extent of infection.
- Numb completely. Local anesthetic—plus laughing gas, oral or IV sedation if you choose. We do not start until you are numb.
- Clean the canals. Through a small opening in the top of the tooth, the infected pulp is removed and each canal is cleaned, shaped and disinfected.
- Seal. The canals are filled with a biocompatible material and the opening sealed.
- Protect with a crown. A ceramic crown—often milled the same day in our office—restores the tooth’s strength and appearance.
Most patients are back to work the same or next day. The toothache is typically gone as soon as the anesthetic wears off.

Does a root canal hurt?
The procedure should not hurt. The tooth and surrounding area are numbed completely, and if you are anxious we add laughing gas, oral sedation or in-house IV sedation. What hurts is the infected tooth before treatment—and a root canal is what relieves it. Mild soreness for a few days afterward is normal and managed with over-the-counter pain relievers.
How do I know if I need a root canal instead of a filling?
A filling repairs decay in the outer layers of the tooth. When decay or a crack reaches the pulp—the nerve and blood supply inside—the tooth becomes infected and a filling won’t fix it. Signs include pain that lingers after hot or cold, spontaneous throbbing, pain when biting, swelling, or a small bump on the gum. Dr. Mady confirms with an exam, X-rays and, when needed, 3D imaging.
How long does a root canal take?
Most root canals are completed in a single visit of 60 to 90 minutes. Molars with several canals may take longer or a second visit. The tooth is then restored with a crown, which we can often design and mill the same day.
Why do I need a crown after a root canal?
A tooth that has had a root canal is more brittle and, on back teeth, likely to crack under chewing forces. A crown covers and protects it so it lasts for decades. Because we mill ceramic crowns in the office, many patients leave with the permanent crown the same day.
Is it better to pull the tooth?
Almost always, keeping your natural tooth is the better long-term choice—it preserves bone and your bite and costs less than replacing a tooth. When a tooth is cracked below the gum or too damaged to restore, Dr. Mady will tell you honestly, and because he places dental implants in the same office, your replacement plan starts right away.
Can you do a root canal the same day I call with pain?
